Hold Harmless/Liability Agreement
I understand that participation in auditions, classes, and rehearsals at Tampa City Ballet involves physical activity, which carries the risk of injury. By signing this agreement, I hereby release and hold harmless Tampa City Ballet, its affiliates, assignees, contractors, lessees, and personnel from any and all liability for injuries, illnesses, or the loss or damage of personal property that may occur during classes, rehearsals, or productions, whether or not such injury, illness, or loss results from the actions of Tampa City Ballet or its employees.
I also acknowledge that performing arts instruction may involve physical corrections, which could include instructors or staff physically touching the dancers as part of normal class or rehearsal procedures.
Visual Representation and Publicity
I hereby grant permission to Tampa City Ballet and its representatives to photograph, record, and/or videotape me and to use these images or recordings for educational, promotional, or publicity purposes. This may include, but is not limited to, printed publications, electronic media, websites, and other forms of communication.
I understand and agree that my name and identity may be revealed in descriptive text or commentary in connection with these images or recordings. I authorize their use without compensation, and I acknowledge that all negatives, prints, and digital reproductions shall remain the property of Tampa City Ballet.
Consent to Medical Treatment
In the event of a medical emergency, I authorize a staff member of Tampa City Ballet to consent to necessary medical treatment on my behalf. Tampa City Ballet and its affiliates will make every reasonable effort to contact the emergency contact listed below prior to providing treatment.
